  • To conduct investigations or breaches of RelianceUnited’s internal policies, laws and regulations, enforce appropriate sanctions and pursue legal actions if necessary.

 

The email address and mobile number Users provide may be used to respond to their inquiries, and/or other requests or questions. If User decides to opt-in to our mail list and/or SMS list, they will receive emails and SMS messages that may include company news, updates, related product or service information, etc. If at any time the User would like to unsubscribe from receiving future correspondences, we include detailed unsubscribe instructions at the bottom of each email and opt-out instructions in our SMS messages, or the User may contact us via our website

 

How will RelianceUnited protect your Personal Data?

 

We adopt appropriate data collection, storage and processing practices and have set up adequate organizational, physical and technical security measures to protect the confidentiality, integrity, and availability of your Personal Data and secure against unauthorized access, alteration, fraudulent misuse, disclosure, destruction and/or any other unlawful processing of your Personal Data stored in our systems.

 

Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. If you have reason to believe that your interaction with us is no longer secure (for example, if you feel that the security of any account you have with us has been compromised), please immediately notify us of the problem by contacting us in accordance with the “Contacting Us” section below.

 

How can you Access, Change, or Delete Your Personal Data?

 

You are entitled to certain rights in relation to the Personal Data collected from you, including the right to access and correct your Personal Data being processed.

 

If you would like to review, correct, update, or delete the Personal Data that you have provided to us via the Website, please contact us via sending an email to dpo@reliancehealth.com.ph. We will try to respond to your request within 24 to 48 hours.

 

Retention Period

 

RelianceUnited and/or its duly authorized third party shall retain the Personal Data for a maximum period of five (5) years counted from the date you provide it to RelianceUnited, or when they were collected, respectively.

 

With respect to Medical Data, we retain such records for at least 15 years from collection. However, depending on specific written agreements, We may retain the data for longer or shorter periods.

This Privacy Statement does not address, and we are not responsible for the privacy, information, or other practices of any third parties, including any third party operating any site or web property (including, without limitation, any application) that is available through this Website or to which this Website contains a link. The availability of, or inclusion of a link to, any such site or property on the Website does not imply endorsement of it by us or by our affiliates.

 

Website Visitor under the Age of 12

 

RelianceUnited Websites are not directed to individuals under the age of seventeen (17), and we request that these individuals not provide Personal Data through the Website.

LEAVE POLICY

This policy defines the leave entitlement of employees and the procedures for administration.

POLICY CONTENT


1. Vacation Leave

 

  • The Company provides the employee the opportunity for rest as well as to attend to other personal necessities. Earned leave/s can be used during the probationary period and is non-convertible to cash.

  • Leave credits are replenished at the start of the year and can be used from 1st January up to 31st of December. Calculations of Vacation Leave for service less than one year shall be made in proportion to the length of service. The employee shall be required to take their annual leave within the calendar year.

  • All leave applications are subject to approval of the immediate superior, and must be filed at least three (3) calendar days prior to the actual leave date. All leaves taken without approval are considered Absence without Official Leave (AWOL) and shall be subject to disciplinary action. However, due consideration will be given to the personal circumstances of employees whenever possible.

  • Disapproved leaves should be rescheduled to other dates. Unused vacation leave credits at the end of year are forfeited and no carryover is allowed.

  • Annual leave may be taken in units of days or half days. Half day duties shall be from 8:30 AM to 12:30PM for morning shifts, and 1:00 PM to  5:30 PM for afternoon shifts. Any duties below the above mentioned hours shall be considered as undertime.

  • At the point of resignation, if an employee has taken more days than her earned entitlement, a proportionate deduction will be made from his/her final paycheck.

  • Annual Vacation Leaves cannot be used to substitute an unauthorised absence or undertime.

  • An employee who is on leave shall, when necessary, sufficiently endorse/turn over to his/her immediate superior or appropriate colleague, any document, equipment, keys, and locks that may be needed by his/her co-employees during his/her absence. He/She shall also advise his/her immediate superior or appropriate colleague of any pending matter that may require attention during his/her absence. Failure to comply with this directive may be subject to disciplinary action depending on the gravity of the damage that may be caused.

  • An employee shall not be allowed to use remaining Vacation Leave credits to offset resignation notice period, otherwise known as “terminal leave”, unless approval has been granted by the Business Unit/Department Head. The employee shall be required to report for work during the 30-day notice from submission of resignation letter. The Business Unit/Department Head must inform HR if this arrangement occurs.

2. Sick Leave

​

  • The Company provides the employee the opportunity to recuperate from illness when needed. Earned sick leave/s can be used during probationary period.

  • All sick leave credits, if unused at the end of the calendar year, are commuted to cash and are paid on the dates determined by the company, usually within December.

  • All unused sick leave credits (pro-rated) are also paid upon separation when an employee resigns with due notice. 

  • An employee is required to present a medical certificate for Sick Leave of three (3) days and more. The employee must endorse the document to the Health and Wellness Officer for Fit to Work clearance and must furnish a copy to his BU/Department Head. Failure to provide the medical certificate on the part of the employee may cause for the leave to be disapproved.

3. ADMINISTRATION

​

  • Employee must file his/her leave application via Sprout HR.

  • Immediate supervisor approves the leave application in the system.

​

4. POLICY REVISION

​

Any change or amendment to this policy must be approved by the Senior Leadership Team prior to its effectivity.

 

5. POLICY COVERAGE

 

This Leaves Policy shall only cover all employees under RelianceCARE, Inc., the holding company.
